Customer fees

An outpatient clinic fee of 46 euros is charged from a person over 18 years of age who is examined or treated at the clinic. If the patient stays at the inpatient ward during the same day, no outpatient clinic fee will be charged. No fee is charged for outpatient psychiatric care or from war veterans. 

A day surgery fee of 150.80 euros is charged for a day surgery operation that is performed in an operating theatre and that requires general anaesthesia, extensive anaesthesia or intravenous medication. A day surgery fee is also charged from those under 18 years of age, unless the procedure has been performed at the outpatient clinic. No fee is charged from war veterans. 

If a person who has undergone an outpatient surgery procedure stays in care to the next day due to a complication or other similar reason, the person is charged a care day fee of 54.60 euros in addition to the day surgery fee. 

The care day fee in psychiatric care is 25.10 euros. 

A care day fee of 54.60 euros is charged for all care days. Once the upper limit of fees has been exceeded, the fee is 25.10 euros. If the patient is transferred directly to another hospital, the day of departure is not invoiced. A care day fee is charged from those under 18 years of age for 7 days in a calendar year, after which inpatient care is free of charge. 

The fee for daytime care or night care is 25.10 euros.   

A serial care fee of 12.80 euros is charged for a maximum of 45 care periods per calendar year from persons over 18 years of age.  

The fee for rehabilitation care is 18.80 euros.

Health centre outpatient fees

The fee for an appointment with a health centre physician is 23 euros. The fee is charged for the first three appointments in a calendar year from those over 18 years of age.

Health centre emergency department fee

  • The emergency department fee between 20.00 and 8.00 on weekdays is 31.60 euros. 
  • On Saturdays, Sundays and public holidays 31.60 euros. 
  • At other times 23 euros. 

The emergency department fee is also charged from a person who has already paid three (3) health centre appointment fees of 23 euros each. 

Unused/uncancelled examination or appointment 56.7 euros. The fee may be charged from a client over 18 years of age on the criteria laid down in the relevant decree. 

Haitari, asiakasmaksut

An upper limit of 762 euros in 2023 has been set for municipal social and health care client fees.  The monitoring period is a calendar year from 1 January to 31 December, and the user of services must monitor the exceeding of the upper limit of fees. 

The client must report the fees collected in another wellbeing services county to the invoicing team of the North Savo Wellbeing Services County. 

If the total amount of fees exceeds 762 euros during a calendar year, the services included in the upper limit of fees are after this free of charge until the end of the calendar year. However, a care day fee of 25.10 euros is charged for short-term institutional care from persons over 18 years of age. 

A certificate of reaching the upper limit of fees is given to the client. The certificate is valid until the end of the year of issue. The fees of persons under 18 years of age can be included in the upper limit of fees of a custodian. 

The following are included in the upper limit of fees:  

  • fees for doctor’s services in outpatient care at health centre 
  • serial care fees 
  • fees for short-term institutional care in health care and social welfare institutions 
  • fees for daytime care and night care 
  • fees for rehabilitation care (care that improves and maintains functional capacity) 
  • oral and dental examination and care fees, excluding technical dental fees 
  • specialised procedures for the mouth and jaws (also at the health centre) 
  • therapies carried out by a health care professional 
    • physiotherapy fees 
    • fees for neuropsychological rehabilitation 
    • nutritional therapy fees 
    • pedicure fees 
    • speech therapy fees 
    • occupational therapy fees and fees for corresponding care that improves and maintains functional capacity 
  • fees for temporary home nursing care and temporary home hospital care, unless the care is part of home care 
  • institutional rehabilitation period fees for medical rehabilitation arranged on the basis of the Act on Special Care for the Mentally Handicapped or on the basis of disability 
  • client fees paid as social assistance 
  • and appointment fees for corresponding remote appointment services in social and health care. 

The upper limit of fees is also increased by services provided as outsourced services, if these are the responsibility of the public health care provider. 

The upper limit of fees is not increased by the following: 

  • fees for ambulance services 
  • fees for medical certificates 
  • fees for laboratory and imaging examinations carried out under a referral from a private doctor 
  • service voucher excess 
  • fees for long-term institutional care 
  • fee for unused appointment (so-called penalty fee) 
  • social welfare services (excluding short-term institutional services) 
  • private health care fees, if they are not the responsibility of public health care 
  • fees paid from statutory insurance 
  • fees collected from persons not resident in Finland up to the costs incurred in providing the service.

Communal housing, regular home care or home nursing  

As of 1 July 2021, the fee to be charged is determined using uniform criteria with the fees for regular home care, i.e. based on the number of service hours corresponding to the client’s needs recorded in the service decision, the client’s ability to pay and the size of the family.  

A fee may be charged for supported housing on the same grounds if the service includes continuous and regular home service or other care and attention. No fee is charged for the support and guidance included in supported housing. 

Long-term round-the-clock sheltered housing and long-term family care (organised under the Social Welfare Act or Health Care Act) 

The fee may not exceed 85 per cent of the customer’s net monthly income after deductions provided for by law. Under certain conditions, the fee may also be determined on the basis of the spouses’ total income.  In this case, the fee is 42.5 per cent of the spouses’ total net monthly income after deductions provided for by law.  

If both of the spouses are in long-term intensified sheltered housing, long-term family care or long-term institutional care, the fee may not exceed 85 per cent of the client’s own monthly income from which deductions in accordance with sections 10c and 10d have been made. The remaining amount available for personal use by the client is at least 182 euros per month. 

Provisions on client fees charged for social welfare and health care services are laid down in the Act (734/1992) and Decree (912/1992) on Client Charges in Healthcare and Social Welfare. Provisions on the reduction and non-collection of client fees are laid down in section 11 of the act, according to which a fee imposed for social welfare services and a fee imposed for health care services according to a person’s ability to pay shall not be collected or it shall be reduced to the extent that the collection of the fee jeopardises the ability of the person or family to subsist or the performance of the person’s statutory maintenance obligation. The reduction and non-collection of the fee shall take precedence over the social assistance referred to in the Act on Social Assistance (1412/1997). 

The reduction or non-collection of the fee applies to all social welfare fees, including transport service deductibles as well as income-related health care fees.

Application for payment relief 

The North Savo Wellbeing Services County has not decided that the equitable fees in health care would be reduced or not collected on the grounds that the charging of the fee would jeopardise the ability of a person or family to subsist or the performance of a person’s statutory maintenance obligation, and not for reasons of dependency. The client is primarily encouraged to make an arrangement for an additional payment period for the invoice or to make a payment plan for paying the invoice. 

You can request the correction of a clear factual error, typing error or calculation error directly, without a written appeal, by calling the telephone number indicated on the invoice or by sending a message to asiakaslaskutus@pshyvinvointialue.fi.  

If, after a clarification, you are still dissatisfied with the invoice, you can require its correction by filing a written rectification request. You have the right to an appeal if the invoice is addressed to you or if the invoice directly affects your right, obligation or benefit. 

Make the rectification request in writing within 30 days of receiving the decision or invoice. You are deemed to have been notified of the decision on the seventh day after the decision or invoice was mailed, unless otherwise proven.  The date of arrival of the invoice is not included in the rectification request period. If the last day of the period is a religious holiday, Independence Day, the First of May, Christmas or Midsummer Eve or a regular Saturday, the rectification request may be submitted on the first workday after it.  

The rectification request must indicate the decision or invoice of which rectification is requested, the type of rectification that is requested and the grounds on which it is requested. Attach the documents you are referring to, unless you have already submitted them. The rectification request must be submitted to the registry office of the North Savo Wellbeing Services County no later than the last day of the rectification request period before the end of office hours.

Residents of member states of the European Union, EEA states or Switzerland must prove their right to immediate medical care by presenting the European Health Insurance Card (EHIC) or a certificate temporarily replacing the European Health Insurance Card, which allows them to receive treatment at the same client fees as residents of Finland. If there is no EHIC, residents living in the Nordic countries can prove their right to treatment by presenting a passport or identity card and by indicating their address in their country of residence. 

The transition period for the withdrawal of the United Kingdom from the EU expired on 31 December 2020, and therefore a client living in the United Kingdom can no longer prove the right to treatment by presenting a passport as of 1 January 2021. Persons coming from the United Kingdom are required to present a new care entitlement card issued by the United Kingdom and valid as of 1 January 2021. In this case, they must also state their address in their country of residence during the appointment. 

Those living in Australia can prove their right to treatment by presenting a passport and by stating their address in their country of residence. The agreement does not apply to students. 

If a person does not present the above-mentioned card or certificate, the person is not entitled to receive treatment with the same client fees as residents of the municipality. Persons from the EU/EEA or Switzerland seeking non-urgent treatment must present a prior authorisation (E112 or S2 form) or they will be charged a fee corresponding to the actual costs. 

Persons coming from other countries must pay fees corresponding to the actual costs for both immediate and non-urgent treatment. In this case, the price to be invoiced is based on the municipal prices of the North Savo Wellbeing Services County. 

Foreigners who have their municipality of residence in Finland or who are covered by Finnish sickness insurance or who have a valid certificate of entitlement to medical benefits in Finland are charged the same fees as permanent residents of Finland. 

A person who does not have a municipality of residence in Finland or who is not treated as a resident of a municipality is charged a fee not exceeding the costs incurred by the municipality or joint municipal authority, unless otherwise provided in an international agreement binding on Finland or in European Union legislation.  

However, no fee is charged: 

1) for vaccinations referred to in section 44 (vaccination included in the national vaccination programme), section 45 (1) (general voluntary vaccination decided by the Government) and section 47 (compulsory vaccination) of the Communicable Diseases Act; 

2) for research to prevent the spread of a generally hazardous communicable disease, urgent treatment referred to in section 50 of the Health Care Act and medicines prescribed for the treatment of a generally hazardous communicable disease; 

3) for the examination, treatment and prescribed medication of a pregnant HIV positive person.  

The above items 1 to 3 do not apply to a person whose stay in Finland is intended to be temporary or who is compensated for the costs under another law or under insurance. 

If the client does not have a municipality of residence in Finland, a certificate of entitlement to care or a certificate of entitlement to care benefits issued by Kela, or if the client is not a traveller in Finland or is not subject to Section 13 of the Client Charges Act, the client may be charged in full for the costs incurred in taking care of the coronavirus.
